School-Based Health Centers
A School-Based Health Center (SBHC) provides comprehensive primary care services and is located directly within a school. There are seven SBHCs in the city. The SBHC program is managed and funded by DC Health.
Physicals for Student-Athletes
DCPS high school student-athletes who do not have an SBHC at their school may enroll at any SBHC to receive a one-time annual well-child visit, with the purpose of clearing the student for sports. This includes the completion of a Universal Health Certificate (UHC) that is needed to participate in athletics. Students should choose whichever SBHC location is most convenient for them. Each SBHC has a different set of enrollment and consent forms that will need to be completed by a parent or guardian. Enrollment forms for each site are linked below. Students must call ahead to schedule an appointment. Appointments for a well-child visit cannot be made through vaccinate.dc.gov – you must call the SBHC directly.
